Dropsy: The swelling is usualy one of the last things you’ll notice, appearing after the fish has already been suffer from internal damage and other subtle signs. You’ll notice it’s a little round in the middle, and assume your fish is pregnant or simply overfed. Next thing you know, its scales is standing on end (hence the name “pinecone” associated with dropsy).
At this point, the damage are likely already done. It’s not a disease. It’s simply the end stage of failing system.
By this point, it’s too late. The fish’s internal organs has started shutting down. Fluids have seeped into your body cavity.
This is the last, outward sign of a lengthy fight inside the fish. When you’re seeing these symptoms, chances are the fish has been fighting an internal battle for days… maybe even weeks. The problem?
The delay. Hobbyists don’t know the window for treating this condition (and others). You can’t wait until it’s obvious to you.
By then it’s usually far more late. And yes I’m talking about most hobbyists; you’ll notice when your fish starts pineconing, and by then you think it’s too late to do anything so you go out and get medication….but it’s not effective because you waited too long! Unless you understand the very first subtle symptoms and intervene quickly, you won’t save the animal.

What Is Dropsy and Why It Is Not a Disease
Bacterial Causes and Stress Factors
This is bacterial. In nearly every case, this isn’t caused by a virus but rather some type of germ (e.g., Pseudomonas fluorescens; Aeromonas hydrophila) that’s simply a part of most any body of water. Because water conditions and a normal functioning immune system typically limit their activity, they’re rarely if ever an issue in healthy tank.
However, as with all things related off the aquarium, stress is the great equalizer. It suppresses the immune system and opens the door for opportunistic organisms. Stress shows up in various ways.
High levels of ammonia/nitrite are poor water parameters that can cause stress. Shock from a recent fish move is also stressful. Aggression from tank mates is stressful.
Another big one is overcrowding. In a bad environment, the fish’s body will divert resources away from maintenance towards survival. This makes them more susceptible to bacterial invasion within their guts and other organs.
The Biological Mechanism of Swelling
Bacteria multiply quick leading to serious internal damage. And that’s not a tumor, it is actualy a type of swelling. That’s edema.
Fluid accumulates because of organ failure. Your organs are failing, specifically your kidneys which no longer filter as they should, and your heart can’t pump blood efficienty either. So the fluids back up in tissues and cause pressure that pushes on skin from below and causes those standing scales.
It’s a biological problem that creates chaos. This causes a mechanical failure. It is like a smoke alarm going off; you know it is there, but you do not see what caused it.
The fire began long before then. The key is early detection and it can be done, even if it is difficult. Do not get hung up on the obvious swellings.
Early Warning Signs to Watch For
Look for other indicators. Look for lethargic behavior. Is your fish hanging out at the bottom, motionless?
No interest in food? Time to check the clamps. Are their fins being held tight against the body instead of flowing out freely?
These are all very subtle indicator. These show distress before the body fails completely. If caught here, then you have a fighting chance.
Treatment and Immediate Intervention
Treat both. First you need to treat the problem, the infection. Second, you need to treat the environment.
For the infection, antibiotics are needed. Typically, people suggest treating with products that contain either Amoxicillin or Kanamycin if it is a gram-negative infection. While medicine can help, it won’t help much unless you get immediate partial water changes.

Prevention Through Water Quality Management
Make sure that their immune system is kept strong. How do you achieve this? By keeping water quality pristine.
Regularly test your water. Ammonia should be 0. Nitrites should be low.
Avoid sudden changes in temperature or pH. Quarantine new fish when possible as they may carry different pathogens. Use a good variety of high quality foods to feed your fish.
This will help with its general health. Minor infections should resolve themselves if your fish is otherwise healthy. So what is the main point?
Don’t wait until you see the pinecone. In most situations, that pinecone will be death sentence. Look for the subtle signs of stress and decline.
Your early warning system is lethargy and loss of appetite. If there’s an issue with water quality, address it before it becomes an emergency. Make sure your fish are comfortable and keep your tank clean.
The Importance of Observation and Care
The lesson from Dropsy is prevention. Prevention rewards observation and it punishes neglect. That’s because dropsy is the last chapter in a tale written by bad luck, or more likely poor care.
Once the scales rise, you can’t change the ending. The story was written long before. What you can do is write new chapters.
Be observant. Monitor your water. Observe your fish.
Often all that stands between life and death are a few days of careful attention. You should of seen it earlier.
